In the model policies, what is the difference between published date, revised date, and reviewed date?

A published date indicates the date that a policy was first created and added to CU PolicyPro.  If model content includes a published date, this means the model content was added to CU PolicyPro on that date and has not yet had any revisions.  Once a revision is made, the published date will be removed and a revised date included.

A revised date shows the last time the model content was changed or updated. 

A reviewed date indicates that the content has not been revised in quite some time, but that the content has been reviewed and is still correct, and does not need a revision.  Not all policies have a reviewed date, just those that have not needed a revision in several years.
